Applying/Reapplying for Chore/Homemaking Services
Only one person per household can be eligible for chore services. To be eligible to receive chore services all applicants must:
1) Complete, sign, date and return the first three pages of our Title XX Chore Services package
2) Have a medical need for services (as verified by a licensed Physician) and supported by an assessment completed by a WCDJFS Social Services worker
3) Fit within the priority guidelines and characteristics listed below and meet the eligibility requirement of being 63 yrs of age or older -OR- eligible for Ohio Medicaid
Priority Guidelines
Only individuals who need services because of specific debilitation or limiting conditions as reflected by a physician statement and the WCDJFS Social Worker’s determination of the presence of at least five of the characteristics listed below in the person’s home. Services must be needed to help maintain the individual and/or their living space in a clean and safe manner. Transportation/errands can be a part of the services, if the provider is willing, but the providers are not required to do so. If services are provided in the form of transportation they have to be less than 20% of the total monthly services provided.
Characteristics Needed To Qualify
1) Bedridden or wheelchair bound
2) Unable to ambulate without the assistance of a walker, cane or crutch both inside and outside of the individual’s home
3) Legally blind
4) Lives alone, no one else present in the home
5) Receives no home maker or nursing services in their home from a public or private source
6) Unable to perform household chores on own and no able-bodied person present in home
7) Unable to drive themselves to medical appointments or grocery/clothing stores and no one present in the home to do so
8) Unable to safely cook or fix meals on their own
9) Takes four or more prescription medications a day, does not include medication that can be purchased over the counter
Disclaimer Statement
The Washington CDJFS will not issue Title XX Vendor payments for services provided by a relative of the individual. To defined a relative for the purposes of this program is as follows: Father, Mother, Sister, Brother, Step-Siblings, Grandfather, Grandmother, Grandson, Granddaughter, Step-Mother, Step-Father, Father/Mother-in-law, Son/Dtr-in-law, Brother/Sister-in-law, Uncle/Aunt by blood or marriage, Cousin, Nephew, Niece, Son, Daughter, Step-children or Spouse.
All applicants will be notified within 30 days of the receipt of application the agency’s decision on Eligibility is in accordance with the Ohio Administrative Code and the ODJFS Regulations. All persons have the right to a county conference and/or State Hearing on the Agency's action or lack of action.
Chore providers are self-employed and are not employees of the State of Ohio or Washington County Job & Family Services. The WCDJFS is not responsible or liable for any loss, damage or related expenses incurred through the performance of these services.
