NON-EMERGENCY TRANSPORTATION (NET)
The Non-Emergency Transportation (NET) program provides transportation or gas vouchers to Medicaid eligible clients to appointments for Medicaid covered services. The services may be requested by phone. After the request is received and eligibility is verified, transportation or gas vouchers are arranged.
Please make transportation or gas voucher requests as far in advance as possible but in any case the request must be no less than (5) business days in advance.
Below are other transportation programs that are available through Medicaid. Your Managed Care Plan's (MCPs) transportation programs should be accessed prior to contacting our agency:
| Care Source | 1-800-488-0134 |
| Molina | 1-800-642-4168 |
| Unison | 1-800-895-2017 |
NET services to and from a medical appointment may be provided to:
| Clients that receive the Ohio Medicaid card |
| Clients eligible for Medicaid through the "spend-down" process and have met their spend-down in the same month of the medical appointment |
Note: Clients must be able to transfer from a wheelchair to a vehicle independently. Vans with lifts are available on a "first come, first served basis".
Note: If you have a Medicaid spend-down card you must meet your spend-down before calling to schedule transportation and you must call at least (5) business days before your medical appointment.
Please have the following information available when you phone in your trip:
| Customer Information | Medical Provider Information |
| Your name | Your doctor's name |
| Your address (including zip code) | Name of your doctor's building/hospital |
| Your phone number | Address of building/hospital (including zip code) |
| Your Social Security Number | Your doctor's phone number |

Note: If you omit any of this information there may be a delay in scheduling your trip. This delay could result in you not receiving your transportation timely.
