assistance you would like to apply for, fill out the form and save it to your desktop. Then scan & email the form to info@wcdjfs.org.
You can also print the form and either fax it to us at (740) 373-9771, put it in the drop-box outside our building or mail it to: 1115 Gilman Avenue Marietta, OH 45750
One of our caseworkers will process your form and get back with you as soon as possible.

- Note: We need to have a phone number on the application so a
casemanager can contact you for the interview. Anyone that does not have a phone will
have to drop off the application and an appointment will be scheduled for you to come
back in for the interview.
